Activities that address the needs of our community

Country SA PHN develops Activity Work Plans for each financial year and submit these for approval to the Department of Health. The plans outline which activities we propose to implement in order to achieve the key objectives of enhancing the effectiveness of health services and improving the coordination of health care.

Our Activity Work Plans detail a broad range of activities that reflect the needs and priorities of country South Australians identified in our needs assessments. The activities outlined in the below Activity Work Plans are aligned with the Department of Health and Aged Care's funding streams.
