The FocusOne Health care finder team are continually working on building strong referral networks with local community organisations across the Riverland, Limestone Coast and Mid Murray regions.
Assertive outreach is a core aspect of the program’s framework, establishing pathways to disseminate program information, which helps to create a seamless referral process.
ac.care invited FocusOne Health care finders to join forces to initiate a community engagement session “Ageing Well – navigating the aged care space”.
Care finder is aimed at people aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islanders). The program is also available to persons aged over 50 years (45 years for Aboriginal and Torres Strait Islander people) on low income, homeless, or at risk of being homeless.
ac.care’s Community Centre’s Community Connections program supports consumers aged 18-64 years to increase their independence in the home and build stronger social and community connections.
During the community engagement session, care finders led interactive discussions about significant support, addressing system barriers, connection and navigation of services and referral processes. Australian Red Cross was also invited to speak on the recent changes in transport costs, addition of social transport and new contact information.
During National Advance Care Planning Week, care finders Kira in the Limestone Coast and Lianne and Belinda in the Riverland spoke to local community groups. They provided essential information about the care finder program and engaged in discussions about the importance of advance care planning and protecting individuals’ wishes.
FocusOne Health work in partnership with Country and Outback Health to deliver the care finder program. Referrals can be made via the website or by contacting (08) 8644 4900.
