What is the care finder program?
The program, commissioned by Country SA PHN, involves dedicated care finders who actively engage with local communities and intermediaries. Their primary goal is to provide intensive support to older individuals, helping them access services like aged care, health, housing, social services, community groups, and mental health support.
Growing Impact
Our data shows that more and more people are benefiting from this program each month. By meeting with a care finder in person, individuals kickstart the process of connecting to these vital services.
Building Strong Partnerships
Helen collaborates closely with commissioned service providers, identifying service gaps and ensuring effective program implementation. We're proud to have forged strategic relationships with organisations like Dementia Australia, Aged Rights Advocacy Services, the Department of Veteran Affairs, Aged Care Volunteer Visitors scheme, Carers SA, and Deaf Connect.
Integration into Healthcare
A key focus for Country SA PHN is the seamless integration of this program into the broader health and aged care systems. Helen is actively engaging with Local Health Networks, Health Advisory Councils, Local Health Clusters, and the Aboriginal Health Council of SA (Elder Care Program).
