The Care Coordination programme has been providing access to health care and specialty services for all patients with chronic conditions. All patients are invited to have an Adult Health Check yearly, with the aim of detecting risk factors early, delivering health promotion messages, identifying chronic diseases early and implementing patient specific management plans. Additionally, the program helps maintain treatment and care management for patients with known diseases.
As always, challenges persist including the remoteness of the area, harsh environmental conditions, staffing complexities and the social determinants of health that affect people’s well-being. Despite this, Nganampa Health Council continues to provide good care for all patients on the APY Lands through a well-coordinated, process driven approach. The prevalence of renal disease on the Lands is high and affects many young people. Currently, there are two Renal Specialist visits a year. This programme helps ensure timely interventions and management for individuals requiring dialysis. Nganampa Health Council can help coordinate this care, providing education, information, and empowering people to make informed decisions about ongoing management. This can involve working closely with tertiary hospital teams, clinic staff on the Lands, patients and their families.
