Country SA PHN’s Primary Care Portal will serve as a streamlined, single point of online interaction for all things related to Country SA PHN. The portal offers a variety of features, including tools for managing your organisation’s relationship with Country SA PHN, accessing health pathways to support patient care, and utilising a library of resources and quality improvement information.

One of the key features is the ability to view your organisation’s service agreements with Country SA PHN, monitor upcoming deliverables, and easily upload those deliverables directly to the portal. Additionally, the portal allows you to request support from Country SA PHN, with the ability to submit, view, and update support requests through the platform.
