Country Wellness Connections: Psychosocial support for your patients

Sonder’s Country Wellness Connections program provides free, non-clinical psychosocial support for individuals aged 16 and over experiencing mental health challenges across the Gawler, Barossa, Adelaide Plains, Light Regional, Wakefield, and Goyder council areas.

With immediate availability, now is the perfect time to connect your patients to this valuable service.

What does Country Wellness Connections offer?

Country Wellness Connections is designed to help individuals build skills, confidence, and independence through:

  • One-on-one coaching with compassionate Mental Health Practitioners and Peer Practitioners.
  • Goal-oriented, person-centred support tailored to each individual’s needs.

  • Quick and seamless referrals – No waitlists, ensuring timely access to support.
  • Complementary to clinical care – Enhances patient outcomes by addressing psychosocial needs to support your patients’ overall recovery and wellbeing.
  • Peer-led, lived-experience approach – Provides relatable, understanding support.
  • Flexible and local – Services are delivered close to home, making access easier for clients.
How to refer

Referrals can be made by GPs, allied health professionals, and other service providers. To refer a patient, please send a referral to the Head to Health Intake, Assessment and Referral service, through fax (08) 9467 6233, HealthLink (EDI: sahthiar) or alternatively through a warm referral via their dedicated phoneline 1800 595 212.
