Pharmaceutical Society of Australia (PSA) is delighted to announce that they are implementing a Dementia Support Pharmacist program to service regional and rural South Australia in the Country SA PHN region.
The program is designed to improve understanding, knowledge, trust, and confidence of medications with clients, families and healthcare professionals living with or assisting those living with dementia. This includes those living within the community, in home settings and residential aged care facilities (however aged care facilities with an Aged Care Onsite Pharmacist (ACOP) are ineligible for the service).
Through face to face and telehealth consults the pharmacists will work with clients, families, specialised nurses, general practices, health professionals, supported living and aged care facilities to provide clinical support to improve client knowledge and confidence in medication management, to improve client quality of care with the aim to support living well with dementia, assist the client to remain in their preferred location for longer whilst maximising independence, promote quality of life and positive relationships, reduce hospitalisations and aid with end of life care should this be needed.
Our pharmacists are experienced, passionate, and caring; working solely to maximise the benefits to those using this service and provide quality person-centered dementia specific care to those living with dementia, their loved ones, and carers. Dee-Anne Hull and Nicola Sander have commenced work with the PSA to engage stakeholders and raise awareness about this worthwhile and exciting project. They are looking forward to receiving referrals and can’t wait to make a difference in the lives of those clients and families they have the pleasure of working with.
