Country SA PHN has collaborated with Healthily to bring to life two additional GoShare bundles to our existing suite of resources.

Adult Attitude to Grief Survey (AAG)
AAG will soon be available to assist General Practice identify and support recently bereaved patients. The campaign will utilise GoShare Plus to send an SMS message to patients with a link to the Adult Attitude to Grief (AAG) tool (for self-reporting). The SMS message will be sent to them from their trusted General Practice or ACCHO.

Palliative Care: Aboriginal and Torres Strait Islander-specific Palliative Care
Bundles are available in GoShare Plus for use by Country SA PHN General Practices and Aboriginal Community Controlled Health Organisations (ACCHOs).

This activity will increase GP and ACCHO access to regionally relevant and culturally appropriate palliative care, Advanced Care Directive, and End of Life education and health literacy for supporting patients/clients; improve PHC workforce delivery of palliative care health messaging for people diagnosed with life limiting conditions, and their carers.
