In an Australian first and in collaboration with Country SA PHN and Preventive Health SA the GoShare Healthy Lifestyle Program has been launched to general practices and Aboriginal Controlled Health Centres throughout country South Australia. The program is a new and innovative way to support consumers with conversations about healthy lifestyle, body image and weight.
Five rural community members talk about their journey while providing advice on the steps they took to better improve their health. Dr Zali Yager from the Embrace Collective, AMA (SA) Chair Dr John Williams and Social Worker Donna Carr talk discuss how health professionals can provide support to consumers in improve their lifestyle.
The program delivers an efficient, targeted, measurable, and culturally appropriate approach to the provision of health and wellbeing information – including a focus on pregnancy and those with chronic disease.
Video messages are sent directly to the consumer’s mobile phone from their primary health care provider with an option to make an appointment to see their GP or Health Professional. The bundles provide links to resources, Preventive Health SA Better Health Coaching website and an activity.
This targeted approach provides consumers with self-management support empowering them to play a more active role in their own health.
The program is funded by Preventive Health SA and Country SA PHN and health professionals can refer consumers to the Better Health Coaching Service website, which is a free evidence based, confidential telephone coaching service to support eligible South Australians to be active, eat well and reduce the risk of chronic disease.
