Did you know that National Palliative Care Week (11-17 May 2025) is fast approaching?

“Every day in Australia, 400 people of all ages die of a terminal illness,” writes Palliative Care Australia. “Everyone one of them could benefit from palliative care – care that is their human right, care that can enrich their final days, and care that too many people still miss out on.”

This year, Palliative Care Australia is asking ‘What’s Your Plan?’, to encourage people in Australia to discuss with those closest to them about “what matters most at the end of life and to make a plan that empowers and respects their choices during the last years, months, and days of life.”

Country SA PHN, in partnership with Palliative Care SA, is proud to support National Palliative Care Week 2025 by distributing Awareness Raising Packs to general practices and residential aged care homes across country South Australia.

These packs are designed to help raise awareness in workplaces and include a range of campaign materials, along with valuable information and educational resources to support staff, patients, residents, and their families and carers.

Look out for the Orange Heart NPCW Poster – we encourage you to hold it up and share a photo on social media during the week to show your support.
