New Clinical Care Coordination (CCC) Service Contracts announced

Country SA PHN (CSAPHN) is pleased to announce the preferred providers for our Clinical Care Coordination services for 2021/22 and 2022/23.

CSAPHN is currently negotiating contracts with these preferred providers and is also working in partnership with all providers to ensure a smooth transition that is as minimally disruptive to clients and providers as possible.

The preferred providers and their service region are:

As of July 2021, CSAPHN will use approximately $3.2 million of Commonwealth funding over two years to enable these organisations to provide Clinical Care Coordination services across the country South Australia region. Services commissioned under this funding are to improve access to adequate level of primary mental health care intervention to maximise recovery and provide wrap-around coordinated care for people with severe and complex mental health needs.

The preferred providers submitted comprehensive plans, community connections and experience in the severe and persistent mental illness field, while promoting sustainability and innovation within their service models.
