Palliative Care Connect is a statewide service that provides information and links to palliative care and bereavement supports in South Australia.
A phone line is available for people with life-limiting illnesses and their family members, friends, and carers, as well as healthcare professionals. Palliative Care Navigators, available through the phone line, provide information, service linkages, and empower individuals to make choices aligned with their culture and preferences.
To talk to a Palliative Care Navigator, Call 1-800-725-548 (PALLI8), available Monday to Friday from 8.30am to 4.00pm.
In addition to palliative care navigation, patients and carers of all ages, community groups, health and aged care professionals, and organisations can connect with grief and bereavement resources and available supports, via a Bereavement Navigation Service, delivered by FocusOne Health.
Bereavement Navigators can help people understand what they are feeling and experiencing and connect them with available supports. The service is available statewide via telephone, telehealth and in person across Adelaide metro, Limestone Coast, Port Augusta, Riverland and Whyalla.
The service will also engage with government and private organisations, community stakeholders, health and aged care providers, and support networks to increase grief and bereavement capabilities, normalise discussion surrounding death and dying matters, and identify and manage grief and bereavement in the community.
