The Palliative Care resource page is a Country SA PHN initiative as a part of the Commonwealth’s Greater Choice for At Home Palliative Care Program (2022-2025).
We aim to support Country SA people, their families, carers, and healthcare professionals, to access palliative & end of life care with a focus on quality, safety, and culturally appropriate at-home services.
Palliative Care Connect is a statewide service that provides information and links to palliative care and bereavement supports in South Australia.
A phone line is available for people with life-limiting illnesses and their family members, friends, and carers, as well as healthcare professionals. Palliative Care Navigators, available through the phone line, provide information, service linkages, and empower individuals to make choices aligned with their culture and preferences.
Palliative Care Navigators also assist general practitioners, palliative care units, other hospital units, aged care and the ambulance service to organise and link people to appropriate supports.

You can also visit the Palliative Care Connect website, which features key information about palliative care and bereavement. It also has a self-assessment tool to help people identify the support and resources they need. A service directory on the website is available to make it easier for South Australians to find relevant palliative care, bereavement and related community services near them.
Advanced care planning allows health professionals to understand and respect a person's future healthcare preferences, for a time when they become seriously ill and unable to communicate for themselves.

Voluntary Assisted Dying - Information and resources regarding voluntary assisted dying in South Australia are available on the SA Health website.
The After Death Audit is intended for PHNs who are engaging, or intend to engage, in quality improvement activities with GPs and their palliative care related data sets. Information from these forms will be used to determine if GP awareness and capability improves with respect to individuals eligible for palliative care, whether changes need to be made in models of care and care provision and assess if integrated and collaborative care is being provided.
The questions are both quantitative and qualitative and should be completed with the support of palliative care providers (e.g., nurses, specialist palliative care providers) where possible. We acknowledge that this option may be particularly helpful in rural/remote areas of work.

Country SA PHN is funding training/events to increase understanding and awareness of palliative, end of life care and death. This initiative is part of the Greater Choice for At Home Palliative Care Program introduced by the Department of Health and Aged Care aimed at improving access to palliative and end-of-life care at home and in the community.
