Practice Network meeting face-to-face Port Lincoln

A face-to-face Practice Nurse Network breakfast meeting was held in Port Lincoln in December 2024 which was attended by 24 nurses from general practice, residential aged care and Eyre and Far North Local Health Network. 

A presentation was provided by Lisa Wilton, Outreach Aged Care Specialist Officer for Services Australia, along with Country and Outback Health and headspace who talked about local services. 

Staff from the Port Lincoln hospital provided an update of the current services, visiting specialists, workforce and discussed referral pathways between the hospital and residential aged care homes.

The meeting not only provided information about services available locally but the opportunity to network and put faces to names.

The Primary Care and Digital Support team along with the Ageing and Palliative Care team provided an update of current Country SA PHN activities.

The feedback was extremely positive, with the aim to hold regular meetings in all regions.

Well organised and awesome to see people face to face, lots of great information to take back to the workplace, with the goal to work together to provide best practice to and for patient care”
