South Australian care finders networking supporting success

Jointly hosted by Country SA PHN and Adelaide PHN, the care finder bimonthly networking meetings have evolved enormously since the care finder program commenced in early 2023. Now, routinely 30 people attend, with some online and many in the room.  

The afternoon kicks off with an education session on a topic the care finders have identified as a knowledge need. Over the last year we have been fortunate to hear from RentRight SA, SeeDifferently (formally RSB), Relationships Australia SA, Elder Care Support Program, the GEM Unit, and also invited attendees from the Department of Health and Aged Care.

A key part of the meeting is sharing complex case studies, with care finders discussing challenges and successes and learning from each other. The meetings and education sessions form a continuous improvement approach to the care finder's role, while also enhancing relationships across the 11 metropolitan and three country SA organisations delivering the care finder program.

Comments from some care finders include, "don't ever stop these meetings", "...so valuable" , "...such a supportive and learning network for us" .
