Aboriginal and Torres Strait Islander Peoples Advance Care Planning, Palliative Care and End of Life Information Bundles 

We are pleased to introduce a valuable resource for general practice and their patients - our educational Aboriginal and Torres Strait Islander Peoples Advance Care Planning, Palliative Care, and End of Life information bundles, now available on the digital Healthily GoShare Plus platform. 

This initiative is supported by four respected local South Australian Aboriginal community leaders: Uncle Major ‘Moogy’ Sumner, Aunty Michelle Jacquelin-Furr, Uncle Adrian Ware, and Margi Sumner. They have graciously provided recorded video messages discussing Palliative Care, Advanced Care, and End of Life planning.  

These messages are thoughtfully put together to encourage open and meaningful conversations with family and loved ones on subjects often considered difficult to address. Drawing on their ancestral wisdom and deep understanding of cultural traditions, these leaders encourage their fellow community members to think about what happens when we reach the end of our days and to plan for it now, to make it easier on us and our loved ones for when the time comes. 

GoShare Plus is a user-friendly online tool designed to simplify the process of delivering relevant educational content to your patients via SMS. There is no cost to your practice for accessing these resources. Country SA PHN is generously funding GoShare until the 30 June 2024, specifically to support general practices in enhancing quality improvement and preventative health activities within country South Australia. 

Discussing end-of-life matters is a crucial step in promoting understanding, acceptance, and preparedness. These conversations may seem challenging, but they are essential for ensuring that individuals' wishes are respected, and their loved ones receive the necessary support during difficult times.
