Supporting those with severe mental illness
The Clinical Care & Coordination (Triple C) program is aimed at providing clinical care and coordination of services for an individual experiencing severe and persistent mental illness. Primarily consultation and liaison may occur with primary healthcare providers, acute health, emergency
services, rehabilitation and support services, family, friends, other support people and carers and/or other agencies that have some level of responsibility for the client’s treatment and/or wellbeing.
