Message from the CEO

The launch of the 24/7 Remote Health Monitoring Service by the South Australian Health Minister Chris Picton was the culmination of a sustained period of work by a determined and skilled group of collaborators – including Country SA PHN, the Rural Support Service, and Integrated Cardiovascular Clinical Network (iCCNet).

The 24/7 service is a brilliant example of an innovative idea being bought to life to support country South Australians to access care when they need it from their place of residence – homes or residential aged care facilities. Read more...

The Palliative Care Resource Page for Health Professionals is a Country SA PHN initiative, as a part of the Commonwealth’s Greater Choice for At Home Palliative Care Program (2022-2025). It directs you to up to date information on various aspects of Advanced Care Planning, Palliative Care and End of Life Care and provides a list of relevant Hotlines.

New Delirium Pathway
Delirium is an acute or subacute deterioration in mental functioning. It occurs commonly in the older population, particularly those in hospitals and aged-care facilities. The cause is usually multifactorial and reversible. Delirium presents as fluctuating alertness (hyper and hypo-arousal), with cognitive impairment that has developed or worsened over hours to days, and is worse at night. Read more...

Breast Cancer Healthy Habits Group
Country SA PHN funds the extremely successful Healthy Habits program delivered by Sonder (in Gawler, Barossa, Yorke and Northern, Eyre and Far North, Flinders and Upper North) and FocusOne Health throughout the Riverland Mallee Coorong, Limestone Coast. Identified need for a Breast Cancer Healthy Habits Group in Clare led to Sonder creating the program in consultation with the McGrath Foundation Breast Cancer Nurse Lindi Brokenshire and coordinated by Optimum Exercise Physiology Senior Exercise Physiologist Lisa Parkinson. SHINE SA Scholarships
Shine SA is offering scholarships for participation in courses and professional development to Aboriginal and Torres Strait Islander focused workers. Priority for these scholarships will be given to professionals who identify as Aboriginal and Torres Strait Islander, work with Aboriginal and Torres Strait Island people or work in an Aboriginal organisation and who have a strong interest in sexual and reproductive health. The Primary Care Support Team hold monthly network meeting providing general practice staff the opportunity to network and discuss matters such as workforce issues. GP shortage, workforce turnover and staff burn-out are issues the Primary Care Team often hear and this information is provided to the Board who are eager to be aware of the current situation in country South Australia.
