Introducing our new Palliative Care Resource Page for Health Professionals Webpage

The Palliative Care Resource Page for Health Professionals is a Country SA PHN initiative, as a part of the Commonwealth’s Greater Choice for At Home Palliative Care Program (2022-2025).

It provides a list of relevant Hotlines and directs you to up to date information on various aspects of Advanced Care Planning, Palliative Care and End of Life Care.

We aim to support Country SA healthcare professionals, to access palliative & end of life care resources with a focus on quality, safety, and culturally appropriate at-home services.
