Greater Choice for At Home Palliative Care

We would like to introduce Janet Richards to the role of Palliative Care Project Officer for the ‘Greater Choice for At Home Palliative Care’ measure. Janet’s background is medical laboratory science, and she has most recently worked with Flinders University as a Research Officer. Her research interests include rural and remote health, resilience, mental health, and medical education. Janet is looking forward to the new experience of working for Country SA PHN to assist in the delivery of this important piece of work.

A new way for clinicians to ensure that patients get care at home
Gather My Crew is an app designed by health professionals for patients who need help at home but don’t know how to ask for it.  The patient downloads the Gather My Crew app from the app store and sends an SMS invite to friends and family. The patient or a support person, then sets-up a list of tasks which are shared with the ‘crew’ via a shared calendar. A support crew member can then accept a task via the app and the patient is assured that they will have help with that task on the day eg transport to appointment.
