Palliative Care Pathways

South Australia’s Palliative Care Strategic Framework (2022-2027) recognises that ‘families, and carers, deserve access to, and receive the best possible end of life and palliative care’. To support this, HealthPathways has developed a suite of palliative care pathways and referral pages with input from key members of the Statewide Clinical Network on Palliative Care, as well as experts in the field across SA including a number of GP Clinical Editors.

In development for some time to ensure input from key stakeholders, the HealthPathways SA team hope these clinical pathways will assist dedicated health professionals working in Palliative Care in South Australia to access to up to date clinical information and navigate services:

Become Involved with pathway development and provide feedback

Health professionals wanting to be involved in HealthPathways SA can do so in a variety of ways:

  • Be involved in the development of a newly created clinical pathway by working collaboratively with the HealthPathways SA clinical team.
  • Provide feedback - HealthPathways are dynamic and your feedback is essential to maintain currency. If you would like to provide feedback, there is a feedback button on every pathway and referral page, or alternatively email us at enquiries@sahealthpathways.com.au.
  • Suggest new pathway areas to be developed in the future; complete suggesting a pathway form and email to enquiries@sahealthpathways.com.au.
