We would like to introduce Sally Patten to the role of Palliative Care Lead for the ‘Greater Choice for At Home Palliative Care’ (GCfAHPC) measure.
Country SA PHN is pleased to be supporting the Commonwealth-funded measure which has been provided to boost palliative care coordination and integration to support people who have a life-limiting condition, by improving choice and quality of care and support in the home. Country SA PHN aims to support country SA people, their families, carers, and healthcare professionals, to access palliative and end of life care with a focus on quality, safety, and culturally appropriate at-home services.
Most recently Sally has been Country SA PHN’s Bushfire Response Coordinator. Sally is a Registered Nurse who has previously worked across palliative care nursing, paediatric oncology, Aboriginal health, and general practice. She has also provided palliative care training in Residential Aged Care Facilities and across community aged care services. Sally has lived PC Carer experience and worked as a funeral director. Sally has also completed a Post-Graduate in Palliative Care at Flinders University.
South Australia’s Palliative Care Strategic Framework (2022-2027) recognises that ‘families, and carers, deserve access to, and receive the best possible end of life and palliative care’. To support this, our HealthPathways SA team in conjunction with key stakeholders, specialists, Statewide Clinical Network and Clinical Editors have developed a suite of Clinical Palliative Care Pathways. Please feel free to access these here as it is hoped that these clinical pathways will assist dedicated health professionals working in Palliative Care in South Australia to access up-to-date clinical information and navigate services.
Country SA PHN has also recently launched a ‘Palliative Care for Community’ webpage, which will soon be followed by a ‘Palliative Care for Health Professionals’ page which is currently being developed.
