SA Virtual care service for aged care providers

The South Australian Virtual Care Service (SAVCS) has been established aiming to provide timely access to high quality specialist care with rapid emergency assessment and pathway navigation enabling 70% of patients to avoid transport and ED presentation and/or admission where appropriate.

Using video telemedicine technology to in-reach into homes including residential aged care facilities (RACFs), the SAVCS team is a multi-disciplinary team of Emergency Consultants, General Physicals, senior nurses and paramedics, connecting you within 15 minutes of a referral. The service works collaboratively with partners across the acute and primary care continuum to deliver coordinated care pathways.

SAVCS has been partnering with Eldercare as a pilot site and early adopter sites including LifeCare, Helping Hand and Premier Health to develop an in-reach service directly supporting RACFs. The service has received great feedback from RACFs and SAVCS clinicians, with safe, quality outcomes for residents. Over 70% of residents have been able to remain in their home with tailored access to out of hospital pathways where needed – improving timely access to quality care and reducing the stress with being transported to a busy Emergency Department.

SA Virtual Care Service are now putting a call out to all RACFs across South Australia to register for the service.
