Unexpected Deterioration in an Older Person

In an older person, a sudden decline in function should be investigated and treated urgently.

Awareness of patient frailty is important because frailty is a distinct health condition wherein the body's systems gradually lose their physiological reserves.  Seemingly minor stressors or changes can produce a disproportionately severe adverse outcome.

HealthPathways SA supports general practitioners and other health professionals with step-by-step assessment and support of health and wellbeing issues experienced by older persons via a new pathway:-

This is an exciting time for the HealthPathways SA team as we further develop the existing Older Persons Health suite of pathways already available on the platform. Currently being reviewed or in development; due to launch later in 2022 are:-

In Review:

  • Cognitive Impairment and Dementia
  • Dementia Management & Support

Coming Soon:

  • Palliative Care
  • Delirium
  • Behavioural and Psychological Symptoms of Dementia
  • Before entering a Residential Aged Care Facility
  • Carer Stress and Support

Become Involved with pathway development and provide feedback

  • Be involved in the development of a newly created clinical pathway by working collaboratively with the HealthPathways SA clinical team.
  • Provide feedback - HealthPathways are dynamic and your feedback is essential to maintain currency. If you would like to provide feedback, there is a feedback button on every pathway and referral page, or alternatively email us at enquiries@sahealthpathways.com.au.
