We are expanding our role in ageing well in place for Senior Country South Australians

As part of the Government’s commitment to Aged Care Reform, Country SA PHN are working with the health sectors and practitioners to improved access to primary care and other health services.

To do this we have established a dedicated Integrated Aged Care Team who will be working with consumers, communities, families, primary care providers, Residential Aged Care Facilities (RACFs), Government and non-government organisations on a range of healthy ageing activities.

We are building on our existing expertise, relationships and understanding of the local health and aged care service needs of our region, and will be commissioning additional healthy ageing activities and interventions to support chronic disease management in senior country South Australians to age well in place or chosen place of residence.

We are providing assistance to RACFs across country SA to have integrated models using virtual consultation facilities that support residents to have greater access to telehealth from primary health care providers in their ‘home’ during and after hours.

Navigating the aged care system can be difficult for all of us and even more so for senior country South Australians and their families. Therefore, we are establishing a network of ‘care finders’ to help navigate the system and ensure that Seniors can access the help they need to age well in place or access aged care supports and timely care.
